Health Insurance
The lifetime maximum limit per covered person is $1,000,000. Eligible medical expenses include:
- Doctor’s office visits
- Hospital services
- Semi-private room & board
- Lab tests
- Pre-admission testing
- Outpatient care/surgery
- Well-child care
- Chiropractic care
Special limitations apply to extended care, pap smears and mammography testing. See your plan brochure for details.
State Standard and Health Savings Plan:
Members are required to notify Medi-Call at 1-800-925-9724 for services defined in the Insurance Benefits Guide. Participation in Medi-Call is mandatory. If you do not notify Medi-Call you may pay a $200 penalty and out-of-pocket limits may not apply. Consult the Insurance Benefits Guide for complete details.
HMO Plans:
Members are required to use participating providers and receive referral for specialty providers.
